Quality CultureとData Integrity

現代では、あらゆる業種で企業の品質に対する姿勢が問われている。
製薬企業においては、品質システムの構築とQuality Cultureの醸成が重要である。

品質保証の移り変わり

FDAがGMPを初めて発出したのが1963年であった。
1900～1950年代は、品質管理の時代といわれている。つまり、試験によって品質を管理していた訳である。
この時代における課題は、
１．試験の精度
２．試験法の妥当性
３．サンプリング
などであった。

医薬品はの試験では、ほとんどが破壊試験であるため、サンプリング検査にならざるを得ない。

しかしながら、サンプリング検査には限界がある。サンプリングされなかった残りの製品等に品質異常があった場合、不良品が出荷されてしまうからである。

バリデーションの考え方は1970年代に米国で発生した大容量注射剤に起因する薬害死亡事故が契機となっている。
問題の注射剤は製造工程で滅菌処理され、出荷試験時には無菌試験に適合していた。
メーカーは試験適合を確認して出荷したが、この製品を投与された患者が死亡する事故が相次いで発生した。
事故の原因は、菌で汚染されていた水を冷却水として使用したため、加熱滅菌処理したバイアルの内部は減圧状態となり、汚染された水がバイアルとゴム栓の隙間を通って内容液を汚染させた。
出荷試験は、無菌試験の抜き取り試験であり、全数を対象として実施していなく、汚染されたバイアルの存在を検出できなかった。
この事故からFDAは、最終製品の品質に注目するだけではなく、製造工程でも品質が確保されていることを保証する必要性に気づき、バリデーションの概念を法規に取り入れることとした。

1978年には、FDAがGMPから現在のcGMPへと移行させた。

1960～1990年代は、品質保証の時代といわれている。
上述のようにGMPにバリデーションの概念が含まれ、それに沿った査察も多く実施され、指摘も出された。
課題としては、
１．Blind Compliance
２．プロセスの科学的な理解
３．Don’t Tell!  Don’t Ask!
などがあげられる。
Blind Complianceとは、規制要件に書かれているから、闇雲に遵守しようとする考え方である。
本来は、プロセスを科学的に理解し、適切な品質保証が出来なければならないのである。
「Don’t Tell!  Don’t Ask!」は、査察の際に、聞かれない限り何も話さない、何も尋ねないといった対応方法である。これによって多くの企業は査察を経験し、指摘の数を少しでも減らしてきた訳である。

その後、QbD（Quality by Design）やリスクマネジメントなどの概念が導入された。

2000年代は、品質マネジメントの時代といわれている。
課題としては、
１．知識管理
２．経営者の責任
３．Quality Culture
等であり、まさに今、Quality Cultureが求められているのである。

企業は品質システムを構築し実践したうえで、組織に知識（知恵）を貯めて行かなければならない。
知識によって、過去の逸脱や事故などの再発を防止し、より高度な品質保証へと移行させるのである。

Quality Cultureとは何か

Quality Cultureの定義は以下の通りである。

  • 患者に良い品質の医薬品を届けようとする組織における、組織・個人全体の態度・信念・行動
  • 組織の中で共有された品質に関わる行動原理や思考様式
  • 品質文化は、その医薬品を製造提供する際に品質レベルを向上させるために企業が採用している一連の態度と価値のこと。

企業は、以下の事項に注意を払う必要がある。

  1. 自社の製造環境に対する十分な理解
  2. 各種規制・ガイドライン、その本質の理解
    規制要件に書いてあるからではダメなのである。
    つまり”No Blind Compliance！”である。
  3. データに基づいた取り組み
    方針の決定は、データに基づくべきである。
  4. 品質改善に対する全社員の積極的な取り組み
    常にプロセスを見直し、改善を行う必要がある。　
  5. 経営者の品質へのコミットメントと評価
    品質システムが機能しているか、従業員の評価は適切かを常にモニタリングし、見直す必要がある。

こうした文化の下で品質保証に対する取り組みが求められている。

なぜQuality CultureなくしてData Integrityはできないのか？

規制当局はQuality Cultureとデータインテグリティの間に相関性があると感じている。
下記のような記事を紹介しよう。

FDAは、2005年から2016年の間にデータインテグリティに関する約225のwarning lettersを発行したが、その中には繰り返されるヒューマンエラーによる逸脱、不十分な教育訓練、システムの不備、システムの認定又は構造、不適切な手順あるいは手順に従わない、そして意図的な改ざん行為が含まれている。データインテグリティに関する指摘の増加により規制当局はデータインテグリティの重要性を再度強調するべく一連のガイドラインを発行することとなった。

Quality Cultureがなければ、データインテグリティの保証はおろか、ヒューマンエラー、交差汚染、逸脱、OOS、回収などの品質問題は後を絶たないであろう。
