医療機器業界では、かなり以前から記録は文書なのか文書ではないのかについて議論がされてきた。
ISO 13485:2016において「文書」には「文書」および「記録」が含まれることが明確化された。
しかしながら4.2.4 文書管理において「ただし、記録は文書の一種ではあるが、4.2.5に規定する要求事項に従って管理する。」とある。
つまり、「記録」は「文書」の一種ではあるが、管理方法は「文書」と「記録」では異なるということである。
一般に規制要件や国際規格などで「文書」と呼ぶと、QMS（品質管理システム）のことを意味する。
記録はQMSを運用することによって作成されることになる。
しばしば「記録」をQMSに含めている企業を見かけるが、適切ではない。何故ならば「記録」はシステム（仕組み）ではないからだ。
では一体「文書」の管理と「記録」の管理では何が異なるのであろうか。
文書の管理
ISO 13485:2016 4.2.4 文書管理には以下の要求がある。
a） 発行前に、適切かどうかの観点から文書をレビュし承認する。
つまり文書はレビュと承認が必須である。
b） 文書をレビュする。また、必要に応じて更新し、再承認する。
ここでいうレビュは「見直し」と解釈する必要がある。文書は定期的に見直し、改訂することが必要である。
c） 文書の現在の改訂版の識別および変更の識別を確実にする。
つまり最新版を識別（通常は最新版のみしか使用できなくする）できなければならず、旧版からの変更履歴が記載されていなければならない。
上記以外にも、配布の管理を確実にしなければならず、廃止文書が誤って使用されないようにしなければならない。
記録の管理
一方でISO 13485:2016 4.2.5 記録の管理には以下の要求がある。
要求事項への適合および品質マネジメントシステムの効果的運用の証拠を示すために、記録を作成し、維持する。
前述の通り、QMSを運用した結果が「記録」である。
なお「記録」は「文書」とは異なりレビュ、承認要求はない。
自社のQMSで適切にレビュ、承認が必要な「記録」と必要がない「記録」を定義しておく必要がある。
例えば、会議の議事録は必ずしもレビュ、承認が必要ではないだろう。しかしながらデザインレビュの議事録の場合は、レビュ、承認が必要かも知れない。
その違いは、当該の「記録」が製品の品質、安全性、有効性に影響するか否かで決定する必要があるということだ。
記録の変更は、識別可能とする。
上述の通り「文書」は定期的に見直さなければならない。しかしながら、「記録」を変更した場合、改ざんになる可能性がある。
したがって、QMSで定義した方法により、正当な理由がある場合のみ「記録」を変更し、変更履歴（電子記録の場合は監査証跡）を記録しておく必要があるだろう。
