改正QMS省令における『実施要領』とは


厚生労働省は、2021年3月26日付で「医療機器及び体外診断用医薬品の製造管理及び品質管理の基準に関する省令の一部を改正する省令」（令和３年厚生労働省令第60号）を公布した。
経過措置期間は 3年間であり、2024年3月26日（改正省令の施行の日から起算して３年を経過する日）には新QMS省令を遵守しなければならない。

改正QMSにおいては「実施要領」という単語が頻回登場する。
では一体「実施要領」とは何であろうか。
その目的は、従前の「取り決め」や「合意書」に代わり「実施要領」を策定することで当該業務の遂行をより確実にするということである。
例として、QMS省令 第5条（品質管理監督システムに係る要求事項）第2項に以下の要求がある。

製造販売業者等は、この省令で文書化することを求められている全ての要求事項、手順、活動及び実施要領を確立し実施し及び維持しなければならない。

上記に対して、逐条解説（薬生発0326第10号） 5．第5条（品質管理監督システムに係る要求事項）関係には、下記のように解説されている。

(4) 第2項の「実施要領」とは、要求事項や手順により求められる特別な取り決めや合意書等の品質管理監督システム（注：QMSのこと）の運用に際して、品質管理監督システム基準書（注：品質マニュアルのこと）やその手順書以外に、運用上必要とされる事項を文書に定めたものをいうものであること。

つまり手順書のみではなく要領書が必要であるということである。
例えば、手順書だけでは適正な製造や検査が困難である。そこで手順書よりも詳細な要領書の作成が必要となった訳だ。
外部委託する際などは特にその重要性が高まることになる。「取り決め」や「合意書」のみでは要求した適切な作業は望めない。

改正GMP省令において「実施要領」の作成が要求されているものに下記のような事項がある。

  • 汚染された製品等の管理に関する実施要領（第25条の2第1項、ISO 13485:2016 6.4.2）
  • 情報等の交換に係る実施要領（第29条第1項、ISO 13485:2016 7.2.3）
  • 設計開発照査に係る実施要領（第33条第1項、ISO 13485:2016 7.3.5）
  • 設計開発検証に係る実施要領（第34条第1項、ISO 13485:2016 7.3.6）
  • 設計開発バリデーションに係る実施要領（第35条第1項、ISO 13485:2016 7.3.7）
  • 製品の監視及び測定に係る実施要領（第58条第2項、ISO 13485:2016 8.2.6）

今後は、従前の「取り決め」や「合意書」に成り代わって、詳細な「実施要領」の作成が強く望まれる。
