EDCシステム利用の考察
1. はじめに
これまで製薬会社は、臨床試験において「紙」の症例報告書（以下、紙CRF）を用いて症例データを取得していたが、最近では電子的にデータを取得するElectronic Data Captureシステム（以下、EDC）が注目されるようになってきた。
症例報告書を電子化できる根拠としては「厚生労働省の所管する法令の規定に基づく民間事業者等が行う書面の保存等における情報通信の技術の利用に関する省令」（厚生労働省令第44号 平成17年3月25日）があげられる。
日本においては、EDCはその利用が始まったばかりである。製薬企業にとっても、規制当局にとっても経験の蓄積がない。
日本国内でのEDCに関する体制・制度が十分に整備されていない現状において、EDCを推進することは、製薬企業および規制当局にとって大きなリスクがあるといえる。
規制当局は、紙CRFを廃止し、電子CRFを原本にした場合、EDCを利用した試験成績が受入れ可能か不明であると述べている。
しかしながら、グローバルではEDCの利用が一般的となり、日本が組み込まれたグローバル治験においてもEDCを利用する機会が増大している。
日本だけがEDC利用を躊躇しているわけにはいかない。
製薬企業は、EDCの安易な運用により今後のEDC推進に悪影響を及ぼさないように、慎重に経験を積んで進めていかなければならない。
2. EDCとは
2.1 RDEとEDC
EDCは、Electronic Data Captureの略であり、本来はソースデータを直接電子的に取得する仕組みを指している。
つまり医療機関の電子カルテシステム等のコンピュータシステムや電子機器と直接つないで、症例データや検査値等を直接取り込むといったものである。
しかしながら現在の臨床試験では、ほとんどが症例データを手入力しているのが現状である。
この形態は正確にはRemote Data Entry（RDE）と呼ばれる。（図1 参照）

2.2 EDC利用のメリット
EDC利用のメリットは言うまでもないが、おおよそ以下の通りである。
- CRF回収までの大幅な業務の効率化、軽減化を実現できる
- 入力時のチェック機能により、クリーニングされたデータの収集ができる
- モニタリング部門からDM部門まで、業務に応じた進捗管理が可能
- 利用者情報、プロトコール情報等が一元管理できる
- コーディングが入力時に行える
- 症例データの固定が早くできる
- 医療機関とモニターが同じ画面でデータを確認しながら、電話で打合せることが可能となる
3. 電子CRFとは

最近のEDCの利用形態は、ASP（Application Service Provider）によるものが多い。これは製薬会社にEDCサーバーを置くのではなく、ASPベンダーサイトにサーバーを置き、医療機関も製薬会社もインターネット等のネットワークを介して利用する形態である。
ASP利用した形態では、基本的には契約の終了とともに電磁的記録がサーバーからCD-R等の電磁的記録媒体に移されることになる。その際に多くの場合、電子CRFはpdf化される。
1症例毎にpdf化された電子CRFには、症例票、変更履歴、電子署名情報が含まれている。（図2 参照）
4. 臨床試験データの電子的取得に関するガイダンスとは
4.1 ガイダンス発行の経緯
「紙」ではなく電子的に臨床試験データを取得する場合には、データの品質及び品質保証の観点で「紙」の症例報告書に劣ることのないよう予め対策を講じておく必要があり、共通の考え方に基づき運用することが望まれる。
電磁的記録を利用する場合の指針として「医薬品等の承認又は許可に係る申請に関する電磁的記録・電子署名利用のための指針」（以下、ERESガイドライン）が施行された。
しかしながら、ERESガイドラインには電磁的記録を利用する場合の全般的な要件が記載されており、症例報告書といった具体的な事例に適用する場合には、より具体的な要件を挙げることが必要である。
そこで日本製薬工業協会 医薬品評価委員会は、2007年11月1日、臨床試験データを電子的に取得する場合の具体的な要件を示すことを目的に「臨床試験データの電子的取得に関するガイダンス」（以下、ガイダンス）と呼ばれる自主ガイダンスを発行した。
4.2 ガイダンスの目次
このガイダンスは、製薬企業がEDCを利用する際や、中央検査機関から電子的に検査値を入手する際の自主基準を定めている。（図3 参照）

4.3 ガイダンスの目的
このガイダンスは、製薬企業がEDCシステムを利用し、症例報告書（以下、CRF）を電子化し、これまでのように紙媒体でCRFを回収する必要のないようにできる要件であるといえる。
すなわちガイダンスは、EDCシステムにより作成されたデータ及び中央検査機関から電子的に入手したデータについて、以下の1)及び2)に示す電磁的記録を医薬品の製造販売承認申請時に利用できる（すなわち電磁的記録が原本と定義できる）ようにするための要件を示している。（図4 参照）
- EDCシステムが管理している電磁的記録
- データ固定後に他の媒体にデータを移した場合の移行先の電磁的記録

一般に電磁的記録の特徴として、紙のように何回コピーしても劣化しないことがあげられる。すなわちどのコピーも原本になり得ることである。そこでガイダンスでは、電子原本の唯一性を要求している。電磁的記録がASPプロバイダーのサーバーからCD-Rに移されることなどを想定する場合、時系列的にどの電磁的記録が電子原本であるかをあらかじめプロトコール等で特定しておかなければならない。
5. EDC利用に関する規制当局の懸念
2007年12月21日にガイダンスの説明会が製薬協によって行われた。この説明会で、医薬品医療機器総合機構 新薬審査第2部の井本昌克氏が講演を行った。
規制当局は“経験の蓄積と慎重な対応によりEDCを推進すること”を求めている。
EDCに限らず、データを電子化することで、関係者の倫理道徳観の欠如等により、不適切な運用が意外に容易に実施され、結果として大きな問題となる恐れが潜んでいる。
たとえEDCシステムが、強固なセキュリティ機能を提供していたとしても、ユーザIDやパスワードの管理を怠った場合、意味がないことになる。
臨床試験の現場、すなわち医療機関等において、誰かの代わりに入力を行う等の安易な代替行為が行われてしまっては、臨床試験データの真正性が確保できないことになる。
規制当局が、EDC利用に際して懸念することは、“データの改ざん等が効率的かつ大規模になるおそれ”があるということである。
過去には紙媒体のCRFでもねつ造や改ざんなどの事件が発生した。
本シリーズでも繰り返し述べてきたように、電子化のリスクは、紙媒体に比べて電磁的記録や電子署名が改ざんされやすくなるということである。
EDCを利用する場合は、強固に改ざんを防止する機能を持っており、またそれら機能を操作する者がセキュリティなどの運用ルールを遵守して利用することが大切である。
また万が一改ざんが行われた場合でも、製薬会社も規制当局もそれを発見できなければならない。
改ざんの発見には監査証跡が有効である。EDCをはじめコンピュータシステムにおいて、監査証跡はあらかじめ定められた手順で参照できることが望ましい。
監査証跡は規制当局にとって改ざんを発見する“最後の砦”であるため、監査証跡自体が改ざんできるようでは、電磁的記録を全く信用し、受入れることができないことになる。
システム特権を使用した場合、直接監査証跡等のデータベースを操作することができ、改ざんが可能となってしまうのである。これを技術的に防ぐことが極めて困難である。どのようなシステムも、運用する側のモラルが問われることになる。
6. FDAのガイダンス
FDAは、2007年5月11日に「Guidance for Industry Computerized Systems Used in Clinical Investigations」と呼ばれるガイダンスを業界向けに発行した。これは1999 年 4 月に発表された「Guidance for Industry Computerized System Used in Clinical Trials」を置き換えるものである。
このガイダンスは、臨床試験でコンピュータシステムを使用し、その結果発生する電磁的記録の取り扱いについてFDAの考えを述べたものである。すなわち電磁的記録の信頼性、品質、完全性の保証を求めている。
21 CFR Part 11（以下、Part11）以降に発行されたガイダンスとして、臨床試験におけるPart11の要件を具体的に記述し、また「Guidance for Industry Part11, Electronic Records; Electronic Signatures Scope and Application」を補足するものとなっている。
EDCシステムの現状をかなり意識した内容になっている。
7. おわりに
現在医療機関で利用されている電子カルテシステムの多くは、バリデートされていないと聞く。
また中央検査機関で使用されている、電子機器やコンピュータシステムのバリデーション実施の状況やレベルも、まちまちである。
データの品質保証は、データソース側が実施するのが原則である。
EDCシステムを製薬会社がバリデートしても、それだけでは臨床試験データの品質保証にはならない。
FDA等の規制当局が日本の臨床試験現場を査察した場合、電子カルテ等のコンピュータシステムがバリデートされていないという指摘を行う可能性がある。
グローバル治験等において、日本の臨床試験データが欧米で受入れられるためには、医療機関、CRO、中央検査機関、製薬会社がそれぞれシステムの信頼性保証を実施しておかなければならないといえる。
