FDAが最も多く指摘しているのは、設計へのインプットを定義していないことである。
設計へのインプットを誤解している企業が多い。一番多くある誤解は、設計へのインプットとはユーザ要求のことであるというものである。また仕様書類は、すべて設計からのアウトプットであるといった誤解である。
設計へのインプットは、決して概念的なものではなく、あらかじめ定義した複数の資料で構成される。
- 設計開発計画書
- 要求仕様書
設計へのインプットを作成する際には、あいまいな要求をを具体的な仕様に落とし込まなければならない。
例えば、「持ち運べること」といった要求のままでは、設計ができない。
そこで、「重さは3Kgまでとする」「一辺が最大10㎝までとする」といったように具体的な仕様に落とし込むのである。
最終的に、設計インプットは適切にレビュ、承認しなければならない。
ここでいう承認は、各資料毎の承認のことだけではなく、設計へのインプットとしての承認である。
つまり、メカ、エレキ、ソフトウェアの要求仕様の組合せとして、問題ないということを承認しなければならない。
また設計へのインプットは、レビュ、承認後、DHFに維持しておかなければならない。
その後に起こる全ての変更は変更管理手順に従う。
FDAのワーニングレターの事例
貴社は21 CFR 820.30（c）、すなわち「設計へのインプット」に適合してない。この設計へのインプットの要件を含む設計管理手順の改訂版がFDAへの提出を約束した期日にまだ届かないため、指摘事項（Form 483）がWarning Letterに発展した。
設計インプットとは
設計インプットは「FDA 21CRF Part820 Quality System Regulation」の「Subpart C§820.30(c) Design input」で規定されている。
| (c)Design input. Each manufacturer shall establish and maintain procedures to ensure that the design requirements relating to a device are appropriate and address the intended use of the device, including the needs of the user and patient. The procedures shall include a mechanism for addressing incomplete, ambiguous, or conflicting requirements. The design input requirements shall be documented and shall be reviewed and approved by a designated individual(s). The approval, including the date and signature of the individual(s) approving the requirements, shall be documented.(c)各製造業者は手順を確立し維持し、機器に関する設計要求事項が適切であり、且つ機器の意図した用途、例えば使用者及び患者のニーズを対象にしていることを保証すること。手順には、不完全な、曖昧な又は矛盾する要求事項を扱うためのメカニズムを含むこと。設計へのインプット要求事項は文書化され、指名された者によって審査され承認されること。承認は、日付、及び要求事項を承認した者の署名を含み、文書化されること。 |
また、設計インプットは同§820.3(f)で次のように定義されている。
| (f)Design input means the physical and performance requirements of a device that are used as a basis for device design.(f) 設計インプット(Design input)とは、機器の物理的及び性能上の要求事項であって、機器設計の基礎として使用するものをいう。 |
なぜ設計インプット要求は重要か？
設計インプットは機器設計開発の開始点であり、設計インプットに関する要求はその後の設計タスクおよび設計の妥当性確認の実施の上での基礎となる。それゆえ、設計開発においてこの要求に関するしっかりとした土台作りをすることは最重要の設計開発コントロール活動である。
もし機器の設計開発に必要不可欠な設計インプット要求が設計の妥当性確認までに識別されていなかった場合には、機器のリリース前に高コストを要しての再設計や作業のやり直しが必要となりうるであろう。
設計インプット要求に従った形での設計開発活動は多大な労力・時間を割かれる。しかし、十分な資源と時間を投入してこの要求を順守する形で設計開発活動を進めることによって、長期的視点で見た時には品質の良い製品の市場への投入による信頼の獲得、結果としてのコストアドバンテージなどを得られるのである。
コンセプト文書と設計インプットの違い
機器を開発する際にはまずその機器のコンセプトを考えるのではないだろうか。マーケティング担当者など、エンドユーザに近い者がユーザニーズを探り、新製品に盛り込むべき機能や機器の形状を考案したり、既存機器の改良を考える。そしてその担当者個人またはチームの思考の結果を文書に落とし込み、新製品の開発または既存製品の改良を提案する。
このような「新製品または既存製品の改良コンセプトを文書化したもの」を設計インプットである考えていないだろうか。残念ながら、これはFDAの品質システム要求が求めるものではない。このようなコンセプト文書のほとんどは機器が備えるべき機能、要件等を包括的に記述したものではないからだ。
FDAの品質システム要求が求めるのは、「機器のコンセプト的な記述を精緻化し、拡大し、エンジニアリングレベルの詳細さの文書にまで落とし込んだ完全な設計インプット要求のセット」（FDA “Design Control Guidance For Medical Device Manufacturers” March 11, 1997）である。
設計開発をするうえで、コンセプトは重要である。ユーザがどういったものを求めているのか、それを記述することは必要である。しかしながら、それだけで機器の開発はできない。例を考えてみよう。「軽い」「持ち運びができる」等、質的な表現だけでは機器の設計開発はできない。こういった単純な質的表現はさまざまな可能性を含有するからだ。重さ、大きさ、耐久性、耐水性、耐熱性等、設計開発を開始するためにはより具体的な記述が必要となってくる。このようにコンセプト文書は設計開発の出発点ではあるが、設計インプット要求ではない。設計インプット要求と言うのは設計開発管理プロセスの第1段階の結果であり、それによって機器の設計ができるものでなければならない。
研究と開発
製品に関する研究段階とその開発段階を区別するのはしばしば難しいことである。研究活動において設計インプットを定義する前に試作品を製作し、その試作品からアイディアの実現可能性を探ったり、設計アプローチを考え出す手法の方が手っ取り早いと考えるかもしれない。しかしながら、このような手法は決して用いてはならない。試作品と完成品は異なるものである。試作品には、完成品に必要不可欠な安全性等の付随機能が欠けているだけでなく、その制作環境も適切なものではないからである。
設計インプット要求事項解説
820.30(c)で医療機器の製造業者が要求されているのは、以下の事項である。
- 設計インプットに関する手順を確立し維持すること
- 手順の確立・維持により、機器に関する設計要求事項が適切であり、且つ機器の意図する使用、例えば使用者及び患者のニーズを対象にしていることを保証すること
- 手順には、不完全な、曖昧な又は矛盾する要求事項を扱うためのメカニズムを含むこと
- 設計へのインプット要求事項は文書化され、指名された者によって審査され承認されること
- 承認は、日付、及び要求事項を承認した者の署名を含み、文書化されること
この章では、これらのFDAの設計インプット要求について1つずつ詳しく見ていく。
設計インプット手順の確立・維持
| 設計インプットに関する手順を確立し維持すること手順の確立・維持により、機器に関する設計要求事項が適切であり、且つ機器の意図した用途、例えば使用者及び患者のニーズを対象にしていることを保証すること手順には、不完全な、曖昧な又は矛盾する要求事項を扱うためのメカニズムを含むこと |
この要求について考える上で「確立する（establish）」という用語を正確に理解しておく必要がある。これについては820.3(k)に以下のように定義されている。
| (k)Establish means define, document (in writing or electronically), and implement.(k)確立する(Establish)とは、定義し、（紙又は電子媒体で）文書化し、且つ実施することをいう。 |
すなわち、FDAの要求する「手順を確立する」というのは、当該作業の各手順内容（誰が何を行うか、どんな記録を残すか）を定義し、文書化（手順書の作成）し、実際に実施する（必要な記録を残す）ことを言うのである。
上記を理解したうえで、設計インプット手順の確立・維持に関する要求について見ていく。
手順により保証すべき事項
FDAは「機器に関する設計要求事項が適切であり、且つ機器の意図する使用、例えば使用者及び患者のニーズを対象にしていることを保証する」ために「手順を確立し、維持すること」を求めている。
すなわち、使用者及び患者のニーズ（意図する使用）を設計インプットとして詳細な要求事項に落とし込み、かつその落とし込んだ要求事項と意図する使用との間で乖離のないことを保証するための手順を確立することを求めている。
前章で述べたように、コンセプト文書と設計インプットは異なるものである。コンセプト文書では「危機に関する設計要求事項」や「機器の意図する使用」を包括的に記述しきれないからである。機器の設計開発を行う者は、機器の備えるべき重要な特性等を提示された時に、それらをエンジニアリングレベルの詳細さにまで落とし込んで文書化するだけでなく、それらに付随してくる対処すべき要素や問題を把握しなければならない。また、機器の設計開発を行う者は、設計の妥当性確認の実施前に、妥当性確認が可能な状態にするためにユーザや患者のニーズを、機器の備えるべき要求事項として詳細に落とし込んでおかなければならない。
機器の設計開発には設計開発部門の人間だけでなく、さまざまな部門の人間が関与する。そのため、機器の要求事項を設計インプットとして詳細にまで落とし込む際には、使用する用語によって個々人間や部門間で誤解が生じることのないようにしなければならない。その際には、エンジニアリング用語に落とし込むことが望ましい。
また、設計開発を行う者の「誤った思い込み」を排除する必要がある。設計開発を行う者が「誤った思い込み」を持ったまま設計開発を行うと、ユーザニーズや意図する使用との乖離が生じ、重大な事象を招く可能性がある。この「誤った思い込み」を排除するために、設計開発を行う者とユーザ要求を代表する者（マーケティング担当者等）が密に連携し、機器設計要求事項を綿密に精査し、記述や記述から想定される内容に思い込みが含まれていないかどうかを調査し、問題がある場合にはそれらすべてを洗い出さなければならない。
手順に含むべき事項
FDAは「手順には、不完全な、曖昧な又は矛盾する要求事項を扱うためのメカニズムを含むこと」を要求している。
設計インプットの具体的な詳細さや範囲は機器の複雑性や使用に関するリスクによって異なる。ほとんどの機器で、機能、パフォーマンス、安全性、規制要件要求事項を含む膨大な要求がなされている。これらの要求はエンジニアリング用語を用い、設計インプットの中で明確化されなければならない。
では、具体的に、どういったことを盛り込まなければならないのだろうか。
FDAによると、設計インプットは下記の3つのカテゴリに分けられる。
機能要件 | 当該機器が何をするものなのかについて、オペレーション上でその機器が可能なことに焦点を当てつつ、設計インプットおよび結果としてのアウトプットを作成について規定したもの。 |
| パフォーマンス要件 | 当該機器が備えるべき量的・質的パフォーマンスを規定したもの。スピード、強さ、反応速度、正確性、オペレーションの限界等の問題について述べること。使用環境の量的な記述（温度、湿度、振動、衝撃、電磁環境適合性等）を含めること。機器の信頼性および安全性も当該要件に含まれる。 |
| インターフェイス要件 | 外部システムとの互換性に重要な機器の特徴について規定したもの。 開発担当者のコントロールの及ばない外部機器との接続に関しては、必ずインターフェイス要件を精緻化すること。すべての機器で重要なインターフェイスはユーザおよび/または患者とのインターフェイスである。 |
すべての機能の特徴やパフォーマンスの特徴を設計インプットステージで確立することが非現実的である場合も数多くある。しかしながら、ほとんどの場合で要求のフォームを決めることは可能である。また、その要求も数値や取り得る値の範囲は後程決定（TBD, to-be-determined）する、と記載しておくことが可能である。そのことによってレビューアは、要求が完全に機器の意図する使用を記述しているかを評価し、欠落していた場合の影響を判断し、問題を確実に解決するために不完全な要求を追跡することが可能になる。
さらに、設計インプット要求は明確なものでなければならない。すなわち、個々の要求が客観的な分析・検査・テスト手法で確認可能でなければならない。
また、設計インプットのセットは矛盾のない、一貫したものでなければならない。
単純なミスで要求間に矛盾が出てしまった場合には、設計開発プロセスの早期に解決する必要がある。
ただ単に、既存の機器のパーツを取り換えただけ、包装やラべリングのみの変更等の場合、詳細な設計インプットを再作成する必要はない。シンプルに関連製品の文書を引用し、新たな製品情報を追加し、包装やラべリングに要求される事項を確立すればよい。
設計インプット要求の適格性評価
最終的に、設計インプットの適格性をレビュしなければならない。レビュ・承認後、設計インプットは管理された文書となる。その後のすべての変更は変更管理手順に則って実施しなければならない。
また設計インプット要求の評価は判断を伴う。そのため、複数の領域に渡る適切な権限を持つレビュチームがレビュを行うということが重要である。
さらに、レビューアが規定された条件が機器の意図する使用条件を表しているかどうか判断することができよう、製品の意図する使用環境を適切に記述しておかなければならない。
設計インプットにおいて業界基準を引用した場合にはその引用が完全であること、また関連のあるものであるかどうかをレビュしなければならない。
設計インプットの変更
大規模な設計開発プロジェクトでは、設計インプットも段階を踏んで作成される。その中でも、たいていの場合は最も困難なのは初期段階における設計インプットの作成である。作成した設計インプットに欠陥があり、変更を余儀なくされることもしばしば起こるであろう。その場合に考慮すべきポイントは以下の2点である。
- 設計インプット要求に関する変更管理プロセスを慎重に管理すること。
- 1つの問題を解決するための設計変更はしばしば他の問題を引き起こす。設計開発活動を通じて、変更による全体の影響を決定できるよう、あらゆる変更が文書化され、設計開発者に伝達されることが重要である。
- 広範囲にわたる設計インプット要求のやり直しは設計インプットが適切な詳細さにまで精緻化されておらず、不適切な資源が要求の定義およびレビュに投じられていることを意味する。経営者はこのことを設計管理プロセスの向上のために使用できる。設計管理の観点から言えば、要求の変更の数は変更管理プロセスを徹底することほど重要ではない。
