医療機器開発の目的は「QOL」の改善である

患者・ユーザーのQOL（Quality of Life）改善のために重要である。
より侵襲度の低い医療機器
より精度の高い医療機器
より使いやすい医療機器
より安全な医療機器
                                            ：

例えば血糖値を測るのにランセット針で毎回血液を採取するものよりも、カフを巻いて侵襲せずに測定できるものがあればよりQOLは改善される。
医療機器の場合は、技術の進歩とともに、その機能や性能が改良されていくことが重要なのでる。
これも患者・ユーザーのQOLの改善である。

品質の4つの種類

品質には以下の４つがある。
①企画の品質: 製品で実現しようとしている特性に対する顧客の要求
②設計の品質: 企画の段階で検討された特性の水準や品質仕様
③製造の品質: 図面・仕様書などの設計文書
④サービスの品質: 調整、据え付け、消耗品の補給、不良品などへの対応に対する顧客の要求

実際の医療現場の方々、医療従事者や患者がどのような医療機器、どのような機能を持った医療機器、どのような性能を持った医療機器を望んでおられるかそういう顧客の要求事項を集めてくることは極めて重要である。
顧客の要求を満たした企画をする、これが企画の品質である。

設計の品質は、企画で要求された特性の水準や品質仕様を満たすように設計をしなければならない。
設計のゴールは２つあり、一つは図面を出して製造できることであり、もう一つは設計資料を出して申請できるようにし審査で承認を取ることである。

製造の品質は、医療機器の場合はたとえ設計が適切であっても製造でもしミスを犯してしまうと品質の悪い医療機器が市場に流れてしまうので、製造の品質を十分に管理する必要がある。
製造は何度繰り返しても同じ企画で同じ品質で安全性を担保した医療機器を製造できなければならない。これが製造の品質である。
逆もまた真であり、製造が適切であっても設計が間違っていれば安全な医療器にはならない。
医療機器においてはこの設計の品質管理と製造の品質管理が極めて重要なのである。

サービスの品質は、例えば調整据付消耗品の補給、不良品などへの対応、定期的なメンテナンス、校正、教育訓練などを言う。
医療機器が医薬品と異なる点は、医薬品は患者が服薬すれば終わるのに対して、医療機器は使用し続けるので、その間に起こる誤使用や経年劣化や故障などに対応し安全性を維持しなければならない。

医療機器の開発で重要なこと　～顧客重視～

トップマネジメントは、適用される規制要求事項に合致するだけでなく、顧客要求事項が決定され、満たされていることを確実にする。」

医療機器を設計するために大事なことは二つあり
一つはユーザーの要求を満たすことと、もう一つは規制要件を遵守することである。
顧客要求は、次の医療機器にはこういう機能が欲しい、こういう性能になって欲しいという、ポジティブな意見になる。
大切なことは、要求された機能や製法を実現するために、どのような安全設計をするかということである。

品質とは

ISO-9000では、品質について「本来備わっている特性の集まりが、要求事項を満たす程度」と定義している。

品質が良いとは？

顧客の要求と製品の仕様が合っている状態、これを品質が良い状態という
使い勝手がいいとか悪いということはあまり関係がない
顧客要求には安全要求がないので、安全要求は設計者が考えなければならない
しかしながら、そもそもユーザー要求が間違っている場合があるので注意が必要である
そこでユーザー環境におけるバリデーションが重要になってくる
例えば、手術する際にメスがなかったと仮定し、とても品質の良い最高級ナイフを買ってきて手術に使えるかというとこれは使えない。この品質の良い最高級ナイフが手術に適しているかというと適していない。これを意図した使用(Intended use)と当該医療機器の仕様が合っていないと事故が起こる。これは品質が良いとは言えない。

医療機器の品質と言うのは、その医療機器そのものの製品の品質のみを規制しているわけではない。
実際の医療現場、実際の患者・ユーザーさんの実際の使用方法に対して、当該機器の仕様が合っているかどうかが大切なのである

これを目的への適合性（fitness for purpose）という
使用目的に対してその医療機器の仕様が合っていないと事故が起きる
すなわちこれは品質が良いとは言えないということになる

Fitness for purpose　意図した使用に“仕様”を合わせる

家庭でプラスネジを締めたいがプラスドライバーは無いとする
プラスネジはマイナスドライバーでも締めることができる
しかしながら、このマイナスドライバーで締めてしまうとネジ穴が潰れてしまう可能性がある。これを繰り返せばドライバー自体が破損してしまう可能性もある

要するに、プラスのネジを締めるという使用目的に対してマイナスドライバーというのは適合していないというわけである
これが問題である

医療機器の品質と言うのは、その医療機器そのものの製品の品質のみを規制しているわけではない
実際の医療現場、実際の患者・ユーザーのお使いなる、実際の使用方法に対して、当該機器の仕様があっているかどうか、これが品質の定義なのである。ここをよく理解頂きたい。 まず企画の品質が大事、どういう要求をされているのか、どういう環境で、どなたが使うのか、実際にどういう使用方法をしているか、ということを理解せずに、医療機器を開発してしまうと、決して品質の良い医療機器にならない、次の話になるが、安全な医療器にはならないということになるので注意いただきたい
